Key Moments in the History of Liposuction by Dermatologists

Abstract
Dermatologists have been pioneers in the development and refinement of liposuction using local anesthesia. Although other specialties routinely use general anesthesia for liposuction, the safety profile of liposuction using local anesthesia is impressive. This article traces the history and development of liposuction by dermatologists in the United States.

INTRODUCTION

Although liposuction began with Dr Georgio Fischer in Italy and underwent further development by Dr Pierre Fournier and Dr Yves-Gerard Illouz in France, many American dermatologists have also made contributions to the development of the procedure.

DISCUSSION

The timeline for liposuction in the early 1980's includes 4 very significant events:

1. A number of American dermatologists, lead first by Dr Richard Glogau in 1981, visited Dr Illouz and Dr Fournier in Paris to observe liposuction. Dr Michele Gasiorowski began liposuction in California after visiting Dr Fournier in April 1982. Drs Claude Caver, Arthur Sumrall, and Saul Asken attended a liposuction workshop given by Dr Illouz and Dr Fournier in mid 1982. Dr Rhoda Narins was the only woman attendee at a liposuction workshop given by Drs Illouz, Fournier, and Fischer in Paris in December 1982. She began liposuction under general anesthesia in the United States (US) shortly thereafter.
2. The first Live Liposuction Workshop sponsored by the American Society of Cosmetic Surgeons (ASCS) /American Society for Liposuction Surgery (ASLSS) took place in Hollywood, California in 1983. Ten dermatologists attended, including Dr Sam Stegman, Dr Ted Tromovitch, Dr Richard Glogau, Dr Saul Asken, and Dr Bill Coleman. Drs Asken and Coleman began liposuction shortly thereafter.
3. A liposuction course was held at Dr Gary Fenno’s surgery center in Houston in February 1985. This course was sponsored by the American Society of Cosmetic Surgery (ASCS)/ASLSS. Dr Jeff Klein, Dr Pat Lillis, Dr Gerald Bernstein, and Dr Bill Cook were among the attendees.
4. Dr Jeffrey Klein began performing liposuction using his tumescent technique in 1985. This allowed liposuction to be done safely on awake patients totally under local anesthesia. Dr Klein lectured on "the tumescent technique" at the 2nd American Academy of Cosmetic Surgery (AACS) World Congress on Liposuction in 1986. Dr Klein published his work initially in the American Journal of Cosmetic Surgery in 1987.1

There were also many other significant events:
The first book chapter on liposuction was published in 1984 in Cosmetic Dermatologic Surgery by Yearbook (eds Stegman SJ, Tromovitch TA).2

Dr Asken published A Manual of Liposuction Surgery and Autologous Fat Transplantation Under Local Anesthesia in 1986.3 He published a second book, Liposuction Surgery and Autologous Fat Transplantation, in 1988.4 These 2 books were the first devoted to liposuction under local anesthesia.

The first safety article "Safety of liposuction: a review of 9478 cases performed by dermatologists" by Bernstein and Hanke was published in 1988 in the Journal of Dermatologic Surgery and Oncology.5 A follow-up article "Safety of tumescent liposuction in 15,336 patients" by Hanke and Bernstein was published in 1995.6

The American Academy of Dermatology was the first specialty society to publish Guidelines of Care for Liposuction in 1989.
