EMPSGC: Why Mohs Might Be the Better Choice

Endocrine Mucin Producing Sweat Gland Carcinoma (EMPSGC) is rare, low-grade, and slow-growing—but it’s not without risk. Though uncommon, recurrence and even distant metastasis can occur, making treatment decisions important despite its indolent nature.

In the recent JDD article, “Endocrine Mucin Producing Sweat Gland Carcinoma Treated With Mohs Micrographic Surgery: A Systematic Review”, the authors compared outcomes between Mohs micrographic surgery (MMS) and traditional excision. Across 27 documented cases, patients treated with MMS had a recurrence rate of just 11.8%, compared to 46.2% for those who underwent traditional excision.

Though the data pool is small and follow-up duration varied, these findings suggest that MMS may offer a more effective approach for managing EMPSGC. For dermatologic surgeons, this article adds to the growing case for Mohs as the preferred method for rare adnexal tumors when tissue conservation and margin control are critical.
