Rapid Clearance of Lichen Planus With Topical Tapinarof

Topical tapinarof associated with rapid resolution of shin lichen planus in a treatment resistant patient

This is a must-read case report in JDD. The report describes a 70-year-old woman with biopsy confirmed lichen planopilaris of the scalp and lichen planus on the shins who had limited response to several topical agents, intralesional steroids, hydroxychloroquine, and topical tofacitinib. After switching therapy, tapinarof 1% cream applied once daily was associated with complete resolution of the shin lesions within one month while the scalp disease remained in remission. The authors note this as the first published case of tapinarof improving cutaneous lichen planus and suggest further study.

As a single case, the observation is descriptive and cannot establish efficacy or guide routine practice, but it may be of interest to practicing dermatology clinicians considering options for refractory lichenoid disease.
