Clinical Efficacy of Second-Generation Tetracyclines as First-Line Systemic Agents for Gingival Lichen Planus

Retrospective JDD study finds minocycline or doxycycline (100 mg twice daily) led to meaningful improvement in inflammatory gingival OLP in two months

In a recent JDD report of 254 biopsy‑confirmed patients whose erythematous or erosive gingival oral lichen planus failed topical therapy, a 2‑month course of doxycycline or minocycline (100 mg twice daily), given alongside topical agents, produced clinical improvement in about 61% of patients. Results were strongest in those with milder gingival inflammation (≈80% improved) and far lower in patients presenting with frank desquamative gingivitis (≈29% improved). Roughly one quarter experienced flare after stopping therapy but most responded to repeat courses. Adverse effects were uncommon and mild (10 patients) and resolved after discontinuation.

This real‑world series suggests second‑generation tetracyclines are a practical, relatively safe systemic option before advancing to immunosuppressive agents for inflammatory, treatment‑refractory gingival OLP.
